Omission of radiation therapy following breast conservation in older (≥ 70 years) women with T1-2N0 triple-negative breast cancer

Haque W, Verma V, Hsiao KY, et al. - Given the randomized data supporting the omission of adjuvant radiotherapy (RT) following breast conservation for T1-2N0 estrogen receptor positive breast cancer in ≥ 70-year-old women, researchers investigated if it could be omitted for triple-negative breast cancer (TNBC). Using the National Cancer Database, they assessed 8,526 patients (≥ 70-year-old) with newly diagnosed T1-2N0M0 TNBC treated with breast conservation. Receipt of adjuvant RT was reported in 6,283 (74%) patients, and not in 2,243 (26%) patients. This investigation was identified to be the largest known such study to date. Finding revealed poorer overall survival in correlation to omission of adjuvant RT for elderly women with T1-2N0 TNBC; this was observed across a range of age groups, as well as following stratification by T stage and chemotherapy usage. They recommend practicing caution when considering the omission of adjuvant RT in node-negative TNBC patients although the results do not imply causation.
