Older age predicts biopsy and radical prostatectomy grade reclassification to aggressive prostate cancer in men on active surveillance

Druskin SC, et al. - Experts evaluated the association between age and biopsy grade reclassification (BGR) to an aggressive phenotype (Gleason score ≥4+3 [GG ≥ 3]). For men on active surveillance (AS), they noted a positive association of older age at time of diagnosis with BGR to GG ≥ 3 and radical prostatectomy grade reclassification (RPGR). AS (as opposed to watchful waiting), for many older men, is a more appropriate management strategy. Findings suggested an association of older age with BGR to GG≥3; as seen on multivariable analysis adjusting for year of diagnosis, race, PSA density and cancer volume at diagnosis. In men who underwent RP following BGR, higher rates of RPGR was seen in older men.
