Obesity at diagnosis and prostate cancer prognosis and recurrence risk following primary treatment by radical prostatectomy

Langlais CS, Cowan JE, Neuhaus J, et al. - In this study, the correlation between BMI—18.5- < 25, 25- < 30, 30- < 35, ≥ 35 kg/m2)— and prognostic risk at diagnosis was examined; the concordance between prognostic risk assessed at diagnostic biopsy vs pathologic risk assessed at surgery across BMI categories was compared; and the correlation between obesity and prostate cancer recurrence and all-cause death was determined. Among men enrolled in CaPSURE (n = 5,200) who underwent radical prostatectomy between 1995-2017, 685 experienced recurrence during a median follow-up of 4.5 years. Observations revealed that BMI is correlated with recurrence following adjustment for disease severity using diagnostic factors (HRvery obese = 1.7); when adjusting for disease severity factors obtained at surgery, this association disappeared. The conflicting evidence regarding obesity's influence on prostate cancer progression may partially be related to residual confounding. In larger men, the possibility of a complicated assessment of T-stage via digital rectal exam may influence clinical treatment decisions. Researchers observed a strong association with all-cause mortality indicating healthier BMI at diagnosis to be related to improved overall survival.
