NT-proBNP level before primary PCI and risk of poor myocardial reperfusion: Insight from the On-Time II trial

Fabris E, ten Berg JM, Hermanides RS, et al. - Researchers sought to determine the potential link between baseline NT-proBNP (N-terminal fragment of the brain natriuretic peptide prohormone) concentration and ST-resolution (STR), a marker of myocardial reperfusion, following primary percutaneous coronary intervention (pPCI) in this post-hoc analysis of the On-TIME II trial (wherein ST-elevation myocardial infarction (STEMI) patients were randomly assigned to pre-hospital tirofiban vs placebo). Cases with measured NT-proBNP prior to angiography were analyzed. In multivariate logistic regression analysis, NT-proBNP, diabetes mellitus, anterior infarct location, time to intervention, randomisation to placebo all were revealed as independent predictors related to higher risk of poor myocardial reperfusion (STR < 70%). Overall, findings demonstrated an independent correlation of higher baseline NT-proBNP concentration in STEMI patients with a higher risk of poor myocardial reperfusion. Based on this observation, the potential use of NT-proBNP was supported as an early marker for risk stratification of myocardial reperfusion following pPCI in STEMI patients.
