Noninvasive oxygenation strategies and all-cause mortality in acute hypoxemic respiratory failure

Ferreyro BL, Angriman F, Munshi L, et al. - By analyzing 25 randomized clinical trials with 3,804 participants, researchers compared the link of noninvasive oxygenation strategies with death and endotracheal intubation among adults with acute hypoxemic respiratory failure. These studies were identified from MEDLINE, Embase, PubMed, Cochrane Central Register of Controlled Trials, CINAHL, Web of Science, and LILACS, all explored from inception until April 2020. The included studies were randomized clinical trials with adult patients with acute hypoxemic respiratory failure comparing high-flow nasal oxygen, face mask noninvasive ventilation, helmet noninvasive ventilation, or standard oxygen therapy. Findings of this network meta-analysis revealed a lower risk of death in relation to treatment with noninvasive oxygenation strategies vs standard oxygen therapy. A lower risk of endotracheal intubation was reported in relation to helmet noninvasive ventilation, face mask noninvasive ventilation and high-flow nasal oxygen.
