Nomogram-based prediction of cervical dysplasia persistence/recurrence

Bogani G, Tagliabue E, Ferla S, et al. - In order to define the factors predictive of cervical dysplasia persistence/recurrence among women undergoing primary conization, researchers undertook this retrospective analysis of 1,503 consecutive patients with high-risk human papillomavirus (HPV) undergoing conization. To estimate the probability of cervical dysplasia recurrence, they developed 2 nomograms: the first depended on the baseline and operative parameters and the second concentrating on type-specific HPV detected. The correlations of HIV infection, positive margins and persistence of HPV with cervical intraepithelial neoplasia 2+ persistence/recurrence were revealed in the multivariate analysis. The first nomogram confirmed the significance of those variables. The influence of type-specific HPV infection in predicting cervical dysplasia persistence/ recurrence was indicated in the second nomogram. The HPV types that most often were related to cervical dysplasia persistence/recurrence were HPV16, HPV18, HPV33, HPV35 and HPV45. For both nomograms, a concordance index greater than 0.70 was generated which is indicative of the reproducibility of these models. The first two nomograms that could predict this risk were proposed in this study. External validation for the documented resulted is needed.
