Nasolacrimal duct management during endoscopic sinus and skull base surgery

Rotsides JM, et al. - During endoscopic sinus and skull base surgery, researchers conducted this retrospective review to assess rates of epiphora after transection and marsupialization of the nasolacrimal duct (NLD). For this investigation, they analyzed records of 29 patients [mean age was 59 years (range, 14-86 years)] from two academic institutions who had endoscopic sinus and skull base surgery with transection of the NLD. According to findings, management varies widely after transection of the NLD. The duct can be transected or marsupialized simply, or a formal dacryocystorhinostomy may be performed. Also, the surgeon must decide whether to place a stent. According to the small series and review of the literature, marsupialization or simple NLD transection results in a low rate of postoperative epiphora in endoscopic sinus and skull base surgery setting.
