Multifocality is associated with central neck lymph node metastases in papillary thyroid microcarcinoma

Zheng W, et al. - In this study involving patients with papillary thyroid microcarcinoma who underwent total or hemi-thyroidectomy with effective unilateral or bilateral central lymph node dissection, researchers investigated how predictive multifocality is to identify patients at high risk of central lymph node metastasis (CLNM). They found that in papillary thyroid microcarcinoma, CLNM was independently predicted by multifocality with ≥ 3 tumor foci. In addition to multifocality, significant associations of age, sex, tumor size, extrathyroidal extension, and nodular goiter with CLNM were also observed. Based on these data, they recommended that the decision for prophylactic central neck lymph node dissection should take into account multifocality assessment. More radical treatment was recommended for patients with multifocality.
