MRI predictors of recurrence and outcome after acute transverse myelitis of unidentified etiology

Bulut E, Shoemaker T, Karakaya J, et al. - Experts recognized MR imaging findings suspicious of relapse and poor outcome in patients with acute transverse myelitis of unidentified etiology. Spinal MRIs of 77 patients diagnosed with acute transverse myelitis were assessed. The recurrent disease gradually developed in 27 patients. Three main important predictors of recurrence were demonstrated, ie, cord expansion, contrast enhancement, and bright spotty lesions. None of the imaging variables exhibited a notable relationship with the disability scores. Hence, in patients with acute transverse myelitis of unidentified etiology, cord expansion, contrast enhancement, and the presence of bright spotty lesions could be utilized as early MR imaging predictors of relapse.
