MRI and a 12-gene expression assay and breast ductal carcinoma in situ treatment

JAMA Jan 21, 2019

Lehman CD, et al. - In a prospective, cohort, nonrandomized clinical trial, researchers examined 339 females to estimate how many converted to mastectomy following magnetic resonance imaging (MRI) and to assess why they opted for those conversions and to assess candidate adherence to radiotherapy guided by the 12-gene ductal carcinoma in situ (DCIS) score. They noted 65 females converted to mastectomy. Among those 65, the basis for conversion was MRI findings in 25, patient preference in 25, positive margins after attempted wide local excision (WLE) in 10, positive genetic test results in 3, and contraindication to radiotherapy in 2. Among 285 who had WLE performed after MRI as the first surgical procedure, ladies achieved successful breast conservation. Ultimately many factors were linked with conversion to mastectomy in women with DCIS who were WLE candidates.
