Mortality, complications and long-term functional outcome in elderly patients with fragility fractures of the acetabulum

Wollmerstädt J, Pieroh P, Schneider I, et al. - Researchers assessed mortality, complications as well as the functional outcome following operative and non-operative treatment of acetabulum fractures in geriatric patients. Participants were individuals aged ≥ 60 years who were treated surgically for low-energy fragility fracture of the acetabulum from 2009 to 2016 and were observed for at least 24 months. One year and 2-year mortality were estimated to be 25.0% and 35.8%, respectively. Death at 1 and 2 years was not influenced by the type of treatment (non-operative vs operative). In-hospital mortality was reported to be 5.7%. More chance of having an in-hospital infection but less probability to sustain thromboembolic events were noted in those who were treated surgically. Overall, geriatric patients with low-energy fractures of the acetabulum experienced high all-cause mortality and in-hospital complications even when treated operatively. In terms of secondary conversion rates to total hip arthroplasty, the outcome was similar compared with younger patients. A fair mid-term functional outcome was observed in those surviving.
