Morphologic features of gastric-type cervical adenocarcinoma in small surgical and cytology specimens

Turashvili G, et al. - Given that misdiagnosis of gastric-type cervical adenocarcinoma (GCA) could be made when well-differentiated examples exhibit bland morphologic appearances, particularly in limited material, researchers sought to characterize the morphologic features of GCA in surgical biopsy and cytology specimens. Patients with histologic diagnoses of GCA or minimal-deviation adenocarcinoma were analyzed. From 23 patients, they reviewed 59 specimens (37 histology, 22 cytology), including histology specimens alone from 6 patients, cytology specimens alone from 4 patients, and both types of specimen from 13 patients. They noted well-to-moderately differentiated adenocarcinomas composed of cells with pale or foamy cytoplasm and well-defined cytoplasmic borders in biopsies. Mild-to-moderate pleomorphism with small nucleoli was evident in nuclei. In a minority of biopsies in which neoplastic glandular epithelium was scant, fragmented, and/or well differentiated, difficulty was faced in making the diagnosis. The submitting pathologist suspected GCA in only 5 (25%) cases among 20 submitted biopsies. Accurate diagnosis of these aggressive tumors in biopsy and cytology specimens might be possible with the awareness of the morphologic features are considered and use of confirmatory ancillary studies (eg, immunohistochemistry for markers of gastric differentiation and human papillomavirus testing).
