Monotherapy for toenail onychomycosis: A systematic review and network meta-analysis

Gupta AK, et al. - Because onychomycosis is a fungal infection of the nail caused by dermatophytes, yeasts and nondermatophyte moulds, which accounts for nearly 50% of all nail-related disease, researchers used a network meta-analysis to evaluate the efficacy and safety of monotherapy and toenail onychomycosis combination treatments. Using Cochrane-compliant rules and the Grading of Recommendations, Assessment, Development and Evaluations approach, quality of evidence was evaluated. Participants in the study were 8,136 patients. According to results, the chances of achieving mycological cure with continuous terbinafine 250 mg or continuous itraconazole 200 mg have been significantly higher compared with topical treatments. Fluconazole, terbinafine and itraconazole pulse regimens and topical treatments were not significantly different in the chances of the mycological cure being achieved. Overall, the authors concluded that toenail onychomycosis oral and topical treatments are safe and effective in mycological cure.
