Monoclonal and polyclonal immunoglobulin G deposits on tubular basement membranes of native and pretransplant kidneys: A retrospective study

Sawada A, Okumi M, Horita S, et al. - Given a link of monoclonal tubular basement membrane immune deposits (TBMID) with interstitial injury progression in renal allograft, and a lack of clarity regarding the significance of monoclonal and polyclonal TBMID in the native kidney, researchers performed this retrospective analysis of 1,894 native kidney biopsies and 1,724 zero‐hour biopsies conducted between 2008 and 2018 in their institution. In native kidney biopsies and in zero‐hour biopsies, the estimated rate of immunoglobulin G (IgG) TBMID was 8.4% and 0.4%, respectively. In 7 cases, presence of monoclonal IgG TBMID was revealed, including three zero‐hour biopsies. Although kidney failure was experienced by one patient with focal segmental glomerulosclerosis, stable renal function was detected in all others. In this study, no link was found between monoclonal IgG TBMID in the native kidney and renal prognosis.
