Missed opportunity encounters for early diagnosis of HIV infection in adolescents

The Pediatric Infectious Disease Journal Jan 11, 2021

Most ZM, Warraich GJ, James L, et al. - Given the significance of early diagnosis of HIV infection for improving patient outcomes and reducing transmission and that adolescents comprise one-fifth of new HIV diagnoses in the United States, researchers aimed at determining the number of missed opportunity encounters (MOEs) before HIV diagnosis for adolescents at a pediatric hospital (PediHosp) and a proximate adult hospital which uses universal HIV screening in its emergency department (ED) (CountyHosp). They conducted an observational study at 2 academic tertiary care hospitals in the United States including 205 adolescents with a new diagnosis of behaviorally-acquired HIV infection. Of these adolescents, 68% were male, 76% were Black and 81% were men who have sex with men. Overall 264 MOE were identified. Observations revealed frequent occurrence of MOE for HIV diagnosis in adolescents. These are greater in number at a PediHosp relative to a similar adult setting with universal screening. Earlier identification of HIV-positive adolescents may be availed with implementation of Universal HIV screening protocols at PediHosp.
