Minimally-invasive management of intramural ectopic pregnancy: An eight-case series and literature review

Shen Z, Liu C, Zhao L, et al. - For preserving fertility in cases with intramural pregnancy (IMP), early diagnosis and management are significant. Researchers here delineated the use of minimally-invasive surgery for early IMP. The clinical data of eight patients with IMP treated at their institution (January 2010 to December 2018) were retrospectively analyzed and the literature describing minimally-invasive treatment of IMP was reviewed. At least one risk factor for IMP was recognized in all eight patients. All cases were successfully managed with surgery (laparoscopic in three patients, hysteroscopic in one patient, and laparoscopic combined with hysteroscopic in four patients), and recovery was reported in all patients without complications. In literature review, 14 articles involving 17 cases of IMP managed with minimally-invasive surgery were identified. Based on outcomes, they suggest laparoscopic surgery as a feasible management strategy for most cases of early IMP. For cases where a sinus connects the gestational sac with the uterine cavity or when cornual ectopic pregnancy needs to be excluded, they suggest hysteroscopic surgery as possibly appropriate.
