Mid-urethral sling revision for mesh exposure - Long term outcomes of two surgical techniques from a comparative clinical retrospective cohort study

Karmakar D, et al. - Researchers compared vaginal closure with vs without sling excision in the management of vaginal sling exposure following mid-urethral sling (MUS). They performed a clinical retrospective cohort study including women with urodynamic SUI who had a MUS [total 2823] during 1999 - 2017 with follow up period up to December 2018. Among these, sling exposure was reported in 33 women (1%) and surgical intervention was undertaken in 31 (1 %). Outcomes revealed the achievement of better outcomes with less recurrence of sling exposure in correlation with sling excision and repair vs simple closure. Of the 20 with primary excision and closure approach, successful closure was achieved in 19. Of eleven with simple vaginal closure without excision, seven required another surgery for recurrent mesh exposure. Further surgery was required in 32% (7/22) who had sling division/removal without a concomitant stress continence procedure.
