Microscopic features of small bowel mucosa of patients with Crohn disease

Cui Y, Lu SY, Xu J, et al. - In this investigation involving 253 consecutive individuals, mean age 35.96 years (14–75 years), who had double-balloon endoscopy and small bowel biopsy between January 2016 and August 2017 at Renji Hospital of the Shanghai Jiaotong University, researchers examined the small bowel mucosal histopathologic characters of Crohn disease and identified some disease-specific changes. The sample consisted of patients without tumors and grouped them into Crohn’s disease, suspected Crohn disease, and non-Crohn disease groups. According to results, on the small bowel mucosa of Crohn disease, there were different forms of granulomatous lymphangitis. They displayed with or without evident obstruction as various macrophages or epithelioid cells in the lumina of lymphatics or in the center of the villi. Such features were only found in patients with Crohn disease. In addition, they were associated with granuloma and lymphangiectasia. Fifteen other characteristics also showed significant variations between the three groups, and Crohn disease patients had an average of almost seven histopathological features. Therefore, it is recommended as a useful histopathological feature for the diagnosis of Crohn disease. It was superior to non-caseating epithelioid granuloma in terms of specificity and sensitivity.
