• This study involved 98 healthy persons and 101 asymptomatic CAS patients, in whom the expression level of serum miR-375-3p was measured.

• By establishing the ROC curve, the predictive value of miR-375-3p for asymptomatic CAS was assessed.

• Asymptomatic CAS patients had significantly higher expression level of serum miR-375-3p vs healthy controls, and the AUC value of ROC curve was 0.888.

• A sensitivity of 80.2 and specificity of 86.7% was obtained, implying high diagnostic value of miR-375-3p for asymptomatic CAS.

• Increased proliferation and migration of vascular smooth muscle cells may result, via downregulation of the apoptosis inducing gene XIAP, due to increased miR-375-3p levels in CAS.
