Metanalysis of transcatheter valve-in-valve implantation vs redo aortic valve surgery for bioprosthetic aortic valve dysfunction

Neupane S, et al. - Researchers analyzed the evidence from studies that reported comparative outcomes of patients who underwent either transcatheter valve-in-valve implantation (ViV-TAVI) or redo surgical valve replacement (redo-SAVR) for aortic bioprosthetic valve (BPV) dysfunction. In this meta-analysis of nonrandomized studies, it was shown that ViV-TAVI, compared with redo-SAVR, had similar 30-day survival, but a higher operative risk as evidenced by significantly higher EuroSCORE I or II. Both groups had similar rates of stroke, myocardial infarction and acute kidney injury requiring dialysis, but the ViV-TAVI group had the lower rate of permanent pacemaker implantation.
