Meta-analysis of valve-in-valve transcatheter aortic valve implantation vs redo-surgical aortic valve replacement in failed bioprosthetic aortic valve

Al-abcha A, Saleh Y, Boumegouas M, et al. - Researchers compared clinical results of valve-in-valve transcatheter aortic valve implantation (ViV-TAVI) vs redo-surgical aortic valve replacement (Redo-SAVR) in failed bioprosthetic aortic valves, via this meta-analysis. They analyzed 12 observational studies with 8,430 patients, and with a median-weighted observation span of 1.74 years. They found a similar risk of all-cause death, cardiovascular death, myocardial infarction, permanent pacemaker implantation, and the rate of moderate to severe paravalvular leakage between ViV-TAVI vs Redo-SAVR. However, the ViV-TAVI group was found to have significantly lower rates of major bleeding, stroke, procedural mortality and 30-day mortality, in comparison with Redo-SAVR.
