Meta-analysis comparing percutaneous to surgical access in trans-femoral transcatheter aortic valve implantation

Abdelaziz HK, Megaly M, Debski M, et al. - Researchers compared results of trans-femoral transcatheter aortic valve implantation (TF-TAVI) done with percutaneous approach (PC) vs surgical cut-down (SC). The outcomes were compared using Valve Academic Research Consortium criteria, in 13 trials with 5,859 patients (PC = 3,447, SC = 2,412). In terms of major vascular complications, major bleeding, perioperative mortality, urgent surgical repair, stroke, myocardial infarction, and renal failure, the outcomes were similar between PC and SC, but PC was related to a shorter hospital stay and less blood transfusion than SC. In PC vs SC, minor vascular complications occurred more often. Overall, findings revealed the safety as well as the feasibility of PC in TF-TAVI as an alternative to SC, and it was noted that the selection of either approach was based on operator experience after ensuring that vascular access could be safely accomplished with that particular technique.
