Meta-analysis comparing percutaneous to surgical access in trans-femoral transcatheter aortic valve implantation

Abdelaziz HK, Megaly M, Debski M, et al. - Researchers compared the results in trans-femoral transcatheter aortic valve implantation (TF-TAVI) done with percutaneous approach (PC) vs surgical cut-down (SC). For this purpose, they analyzed 13 trials with 5,859 patients (PC = 3,447, SC = 2,412). In these trials, a comparison was performed for the results, based on Valve Academic Research Consortium criteria, between PC and SC in TF-TAVI. Similar outcomes were reported in relation to PC vs SC in terms of major vascular complications, major bleeding, perioperative mortality, urgent surgical repair, stroke, myocardial infarction, and renal failure, however, PC vs SC was found to be associated with shorter hospital stay and less blood transfusion. Overall, findings revealed safety as well as the feasibility of PC as an alternative to SC in TF-TAVI, and the use of either approach relies on operator experience following ensuring that vascular access could be safely accomplished with that particular technique.
