Meta-analysis comparing outcomes of drug eluting stents vs single and multi-arterial coronary artery bypass grafting

Gaudino M, et al. - Researchers performed this pairwise and network meta-analysis to analyze data from randomized trials and adjusted observational studies comparing the benefits of coronary artery bypass (CABG) using single and multiple arterial grafting (SAG, MAG) and drug eluting stent (DES) in multivessel coronary disease, with long-term mortality being the primary endpoint and operative mortality, perioperative stroke and follow-up repeated revascularization (RR) being the secondary endpoints. Studies with ≥1.7 arterial grafts/patient were categorized as MAG. The analysis of a total of 53,239 patients (8 randomized, 17 observational studies; 26,306 DES; 26,933 CABG) revealed an association of CABG vs DES with reduced 5-year mortality and need for RR. Based on the benefits seen on primary and all secondary outcomes, MAG was ranked as the best treatment. For studies with higher mean number of arterial grafts, a non-significant trend toward lower long-term mortality was seen.
