Ibrahim J, et al. - This study entails characterization of the medication discrepancies in late-stage chronic kidney disease (LS-CKD). Findings reported that medication discrepancies were common in LS-CKD, impacted the majority of patients and included high-risk medication classes. Data also reported an independent association of congestive heart failure and total number of medications with greater risk for multiple drug discrepancies. Furthermore, a need for great care in medication management was realized in view of the frequency of medication discrepancies.
