Mechanism and extent of myocardial injury associated with out-of-hospital cardiac arrest

Berden J, et al. - Researchers examined 159 consecutive patients undergoing immediate coronary angiography following out-of-hospital cardiac arrest (OHCA) to determine the mechanism and extent of myocardial injury associated with OHCA. Patients were divided into groups with acute culprit lesion (A), stable obstructive coronary disease (B) and non-obstructive or absent coronary disease (C). Findings revealed that, in the presence of acute culprit lesion, significant myocardial injury associated with OHCA may occur. In stable or absent coronary disease, the extent of the myocardial injury is significantly smaller and corresponds with the duration and intensity of cardiac resuscitation. Prediction of the presence of acute culprit lesion with admission cardiac troponin I (cTnI) in combination with post-resuscitation electrocardiogram was not identified as sufficiently accurate.
