Maternal cardiac function at 19–23 weeks’ gestation in the prediction of gestational diabetes mellitus

Gibbone E, Wright A, Campos RV, et al. - In this study, maternal cardiovascular indices at 19‐23 weeks’ gestation were compared between pregnancies that develop gestational diabetes mellitus (GDM) and those without GDM. In addition, researchers examined if the cardiovascular alterations are the consequence of maternal demographic characteristics and medical history or the GDM per se. In this prospective observational study, the study population comprised 2,853 pregnancies containing 199 (7.0%) that developed GDM. Following were the three main findings of the study: first, pregnancies developing GDM were significantly different from the non‐GDM group in E/A, E/e’, myocardial performance index and global longitudinal systolic strain; second, peripheral vascular resistance and myocardial performance index were the only cardiovascular indices that were significantly different between the GDM and non‐GDM groups and they were both marginally increased after adjustment for maternal demographic characteristics and medical history known to affect cardiac indices; and third, no improvement in the performance of screening for GDM by maternal demographic characteristics and medical history occurred by the addition of cardiovascular indices.
