Maternal and neonatal outcome after vaginal breech delivery at term after cesarean section – A prospective cohort study of the Frankfurt breech at term cohort (FRABAT)

Paul B, Möllmann CJ, Kielland-Kaisen U, et al. - Researchers compared women with a prior cesarean section with primiparous patients with regard to the neonatal and maternal outcomes. Further, they compared the mode of delivery of intended vaginal breech deliveries between the two groups of women. They assessed 37 women with a prior cesarean and 567 primiparous women who presented for an intended vaginal singleton breech delivery at term between January 2007 and December 2016. Successful vaginal delivery was reported in 19 and 344 women, respectively, in the two groups. Women with prior cesarean vs primiparous patients did not report significantly different neonatal morbidity and mortality as well as maternal outcome following successful vaginal deliveries. Further, no significantly higher cesarean section rate was reported in the group of women with a prior cesarean (49%) vs the group of primipara (39%). These finding do not support excluding women with a prior cesarean from a planned vaginal delivery due to a breech presentation at term.
