Masked uncontrolled hypertension is not attributable to medication nonadherence

Siddiqui M, et al. - Researchers investigated antihypertensive medication nonadherence as a cause of masked uncontrolled hypertension (MUCH) in this study involving 184 patients who were confirmed to have controlled office blood pressure (BP). Adequate 24-hour ambulatory BP recordings were available for 167 patients. In-clinic BP assessment was used to control 86 of 167 patients, but these patients had uncontrolled ambulatory awake BP, which was indicative of MUCH. Controlled in-clinic and ambulatory awake BP, consistent with true controlled hypertension, was detected in the remaining 81 patients. A similarly high level of antihypertensive medication adherence was shown via analysis of urinary drug and drug metabolite levels in both MUCH and truly controlled hypertensive patients. Based on the findings, the investigators concluded that antihypertensive medication nonadherence is not responsible for MUCH.
