Management of early hepatocellular carcinoma: Results of the Delphi consensus process of the Americas Hepato-Pancreato-Biliary Association

Gholami S, Perry LM, Denbo JW, et al. - As patients with early-stage hepatocellular carcinoma (HCC) have many possible treatment options and practice trends differ widely, researchers in this project wanted to find consensus regarding the management of small resectable HCC using a Delphi conference. A base case was developed with an analysis by the members of the AHPBA Research Committee. To find areas of agreement and disagreement, the Delphi panel of experts reviewed the literature and scored clinical case statements. Consensus an agreement or disagreement was reached on 27 statements by the panel. Including resection, ablation, transplantation, or any locoregional therapy as a bridge to transplant were all considered suitable modalities for early or recurrent HCC. Consensus was reached regarding larger lesions that radiofrequency ablation and microwave ablation were not suitable therapies. An expert panel concluded that for early-stage HCC, multiple treatment modalities are suitable, using a validated system for identifying consensus. These consensus guidelines are meant to help guide clinicians through early HCC treatment modalities, but on a patient-specific basis, clinical decisions can continue to be made.
