Management of comminuted patellar fracture fixation using modified cerclage wiring

Sun Y, et al. - Researchers examined the efficacy and safety of a fixation technique for comminuted patellar fracture fixation using modified cerclage wiring. They assessed outcomes of 38 cases (males: 16; females: 22; aged 23–68 years) of simple unilateral closed comminuted patellar fracture treated by modified cerclage wiring from February 2016 to April 2018. Based on the AO/OTA classification, comminuted patellar fractures were classified: 10 cases were type 34-C2 (three fragments), 28 cases were type 34-C3 (more than three fragments). During a follow-up of 6–36 months (mean time 16.1 ± 5.8 months), satisfactory results were obtained according to the clinical grading scales of Böstman, and the mean score of 28.7 (range 20–30) points was recorded at the final follow-up. Outcomes support modified cerclage wiring as a new effective method for managing a comminuted patellar fracture. This technique seems to have the potential in rendering strong and stable fixation, empowering patients to perform an early functional exercise and have a good clinical effect.
