Malnutrition in acute heart failure with preserved ejection fraction: Clinical correlates and prognostic implications

Chien SC, Lo CI, Lin CF, et al. - Among postdischarge Asians with heart failure with preserved ejection fraction (HFpEF; n = 1,120, mean age: 77.2 ± 12.6 years, 39.4% men), researchers assessed the prognostic value of nutritional status. They assessed serum albumin (SA), prognostic nutritional index (PNI), Controlling Nutritional Status score, and geriatric nutritional risk index to estimate nutritional metrics. Independent associations with malnutrition of lean body size, higher white blood cell counts and C-reactive protein levels, anaemia, and lack of angiotensin blocker use were evident. Longer survival was observed in relation to higher SA levels, higher PNI, and higher geriatric nutritional risk index. Better nutrition with increasing body mass index provided a marked survival benefit, as revealed by restricted cubic spline regression. Findings revealed a frequent and strong link of malnutrition with systemic inflammation in Asian patients hospitalized for acute HFpEF. Additionally, a possible pivotal role of nutrition in metabolic protection in this population was suggested.
