Magnitude of the difference between clinic and ambulatory blood pressures and risk of adverse outcomes in patients with chronic kidney disease

Ku E, et al. - Among 610 participants of the AASK (African American Study of Kidney Disease and Hypertension) Cohort Study, researchers assessed death and end-stage renal disease in relation to the absolute systolic blood pressure (BP) [SBP] difference between clinic and awake ambulatory BPs (primary predictor), by using Cox models. They found a U-shaped association between the magnitude of the difference between clinic and ambulatory SBP and mortality. In black patients with chronic kidney disease, a higher risk of mortality could be seen in relation to higher clinic vs ambulatory BPs (as in white-coat effect).
