Magnetic resonance imaging-based validation of the 2018 FIGO staging system in patients treated with definitive radiotherapy for locally advanced cervix cancer

Kim J, Cho Y, Kim N, et al. - Modern imaging modalities including magnetic resonance imaging (MRI) and PET-CT are now incorporated in the 2018 International Federation of Gynecologic and Obstetrics (FIGO) staging system. Researchers here aimed at validating the revised 2018 FIGO staging system in 677 patients diagnosed with pelvic MRI and treated with definitive (chemo-)RT for locally advanced cervix cancer (stage IB2/IIA2-IVA or N+) between 1992 and 2018. In 392 (57.9%) patients, they observed stage migration between 2009 FIGO and 2018 FIGO staging. A nomogram was developed that may aid in improving prediction of progression-free survival after RT. Among FIGO III patients, poorer survival was noted in correlation with pelvic wall invasion vs pelvic or paraaortic LN metastases. Better performance was reported for the nomogram incorporating FIGO staging relative to 2009 FIGO or 2018 FIGO in predicting progression-free survival.
