Lymph node ratio as valuable predictor in pancreatic cancer treated with R0 resection and adjuvant treatment

You MS, Lee SH, Choi YH, et al. - Given the importance of lymph-node (LN) metastasis as a prognostic factor in resected pancreatic cancer, researchers determined whether AJCC 8th edition N stage, lymph-node ratio (LNR), and log odds of positive lymph nodes have prognostic value in resected pancreatic cancer. Participants were 351 patients with pancreatic cancer who had received R0 resection and adjuvant therapy between January 2005 and December 2017 at Seoul National University Hospital. They found significant correlations of the three LN classification systems with overall survival (OS) and recurrence-free survival (RFS) in log-rank tests and multivariate-adjusted models. The highest discrimination ability in predicting OS and RFS was displayed by LNR, on internal validation. The highest C-index was displayed by LNR in subgroup analysis, classified by adjuvant treatment modality. Overall, the best prognostic performance of LNR, as well as its significant link with loco-regional recurrence demonstrates its ability to serve as an aid to further stratify the patients and ascertain an active treatment plan.
