Lorazepam timing for acute convulsive seizure control (LoTASC)

Kamdar HA, Hamed M, Smetana KS, et al. - This retrospective study was undertaken to determine if administering lorazepam for convulsive seizures lasting less than 5 minutes causes vital instability. The sample consisted of patients who presented with a seizure lasting less than 5 minutes between 2011 and 2016. Continuous variables of lorazepam receivers vs non- receivers have been analyzed using t-test for parametric and Mann-Whitney U test for nonparametric data. Out of 1,052 participants initially screened, 165 were involved in the study. In patients who received lorazepam, there was a significantly greater incidence of vital instability (defined as receipt of a vasopressor or intubation), and this effect was dose-dependent. Lorazepam administration significantly lengthened the length of stay in the intensive care unit. Findings indicate that administration of lorazepam for acute convulsive seizures that do not meet the requirements for convulsive epileptic status can result in iatrogenic vital instability and ICU admission.
