Long term survival and perioperative propensity score matched outcomes of diaphragmatic resections compared to stripping in cytoreductive surgery + intra-peritoneal chemotherapy

Singh B, et al. - Researchers investigated how diaphragm resection and repair in cyto-reductive surgery (CRS) and intra-peritoneal chemotherapy (IPC) influence the short and long term outcomes. They analyzed 1230 consecutive CRS/IPC procedures performed in Sydney, Australia; diaphragmatic intervention was undertaken in 652 of these. Comparison of diaphragm interventions to non-diaphragmatic CRS as well as comparison of diaphragm resection to diaphragm stripping was done. Outcomes indicate the essentiality of diaphragm interventions in CRS. Increase in perioperative morbidity was observed in correlation to these interventions. Patient undergoing diaphragm stripping or resection showed no difference in this morbidity incidence. However, a necessity for diaphragm resection seems to be an indicator of tumor aggression in mesothelioma and LAMNs.
