Long-term risk stratification of patients undergoing coronary angiography according to the Thrombolysis in Myocardial Infarction Risk Score for Secondary Prevention

Zafrir B, et al. - In real-world patients (n=13,593) presenting for coronary angiography, researchers tested Thrombolysis in Myocardial Infarction Risk Score for Secondary Prevention [TRS2P; a risk score based on 9 established clinical factors and meant for secondary prevention post-myocardial infarction] with respect to its performance for predicting long-term outcomes. In this retrospective analysis, they carried out risk stratification for 10-year major adverse cardiovascular events, using the TRS2P, classified into 6 categories (0 to ≥5 points), and in relation to the presenting coronary syndrome. A graded manner rise in the annualized incidence rate of major adverse cardiovascular events was seen with rising TRS2P, ranging from 1.65 to 16.6 per 100 person-years. With C-statistics of 0.693 and 0.758 for major adverse cardiovascular events and for mortality, respectively, reasonable discrimination was shown by TRS2P. In patients undergoing coronary angiography, it was possible to perform risk stratification by using TRS2P, a simple risk score based on routinely obtained variables. In a real-world setting with long-term follow-up, TRS2P was shown to have predictive value, irrespective of the acuity of coronary presentation.
