Long-term outcomes of Holmium laser enucleation of prostate and predictive model for symptom recurrence

Droghetti M, Porreca A, Bianchi L, et al. - In patients with benign prostatic hyperplasia (BPH), durable symptom relief can be achieved with Holmium laser enucleation of prostate (HoLEP), irrespective of the technique used.
A retrospective analysis of 567 patients that consecutively had HoLEP for BPH from 2012 to 2015 at two tertiary referral centers.
Median prostate volume was 80cc, with median peak flow rate 8 ml/s and median post voidal residue (PVR) 100cc.
At follow-up, 125 (22%) patients had symptoms.
A total of 25 (4.4%) patients required redo surgery.
Post-adjustment for likely confounders, an increase in preoperative PVR (odds ratio [OR] 1.005) and International Prostate Symptom Score (OR 1.12) were identified as independent predictors for symptom recurrence.
Cases with a significant preoperative symptom burden or a high PVR should be carefully counseled regarding the risk of symptom recurrence.
