Long-term outcome of primary bilateral macronodular adrenocortical hyperplasia after unilateral adrenalectomy

Oßwald A, et al. - In this retrospective observational study, researchers analyzed long-term clinical and biochemical outcomes of unilateral adrenalectomy vs bilateral adrenalectomy in primary bilateral macronodular adrenocortical hyperplasia (PBMAH) patients vs results of cortisol-producing adenoma (CPA) treated by unilateral adrenalectomy. For this investigation, they included 25 PBMAH patients after unilateral adrenalectomy (unilat-ADX-PBMAH), 9 patients with PBMAH and bilateral adrenalectomy (bilat-ADX-PBMAH) and 39 patients with CPA and unilateral adrenalectomy (unilat-ADX-CPA). According to findings, unilateral adrenalectomy of PBMAH patients results in clinical remission and lower incidence of adrenal crisis, but less biochemical control of hypercortisolism might lead to higher mortality. Deaths occurred only in patients who, after unilateral ADX, were not biochemically controlled.
