Long-term outcome of myotomy and fundoplication based on intraoperative real-time high-resolution manometry in achalasia patients

Triantafyllou T, et al. - This is the first study introducing the use of high-resolution manometry (HRM) during surgical treatment for achalasia and evaluating the long-term outcome of this technique. In the period from November 2013 to July 2016, patients with achalasia were recruited who had HRM and evaluated Eckardt scores (ES) before and after tailored laparoscopic myotomy and fundoplication with intraoperative recording using HRM. The results of this prospective study indicated that the use of intraoperative HRM gives us the advantage of simultaneous real-time estimation of esophageal intraluminal pressures and the ability to determine the exact pressure points during laparoscopy. It can, therefore, be the key to tailoring the Heller-Dor technique and improving the results for patients with achalasia. Overall, they concluded that intraoperative HRM could be a useful tool for safely tailoring the myotomy and fundoplication to achieve the optimal functional and clinical result.
