Long-term outcome of MR-guided Focused Ultrasound treatment and laparoscopic myomectomy for symptomatic uterine fibroids

Mohr–Sasson A, et al. - In this cohort study, researchers compared the long-term outcome of laparoscopic myomectomy to MR-guided Focused Ultrasound (MRgFUS) for symptomatic uterine fibroids. MRgFUS treatment or laparoscopic myomectomy for uterine fibroids led to comparable satisfaction regarding long-term outcome and rate of reinterventions.
Methods
- This cohort study assessed all patients with symptomatic uterine fibroids admitted to a single tertiary care center who were treated operatively with laparoscopic myomectomy or treated conservatively with MR-guided Focused Ultrasound from January 2012 to January 2017.
- Using the Uterine Fibroid Symptom and Quality of Life symptom severity score (UFS-QOL-SSS), researchers assessed further interventions and sustained fibroid-associated symptoms.
Results
- A total of 154 women met eligibility criteria and were enrolled in the study.
- The investigators achieved complete follow-up for 64 women who underwent laparoscopic myomectomy and for 68 women who were treated by MRgFUS.
- The two groups were followed up for similar time period (median [interquartile range (IQR)]: 31 months [17-51] vs 36 months [24–41]; P=0.95).
- The respective rate of additional interventions was 5 (7.8%) and 9 (13.2%) (P=0.312).
- Similarly, at follow-up interviews, the UFS-QOL-SSS questionnaire score revealed comparable median scores: 17(12-21) vs 17 (13-22) for laparoscopic myomectomy and MRgFUS, respectively (P=0.439).
- These findings remained unchanged on separate analysis of each of the symptoms (bleeding, changes in menstruation, abdominal pain, bladder activity, nocturia, fatigue) and on a multivariate analysis.
