Long-term clinical outcomes of Amplatzer cardiac plug vs Amulet occluders for left atrial appendage closure

Kleinecke C, Cheikh-Ibrahim M, Schnupp S, et al. - Researchers retrospectively analyzed three Amplatzer registries (Bern, Coburg, Zurich) to compare long-term clinical results following left atrial appendage closure with the Amplatzer Cardiac Plug (ACP) and Amulet. In these registries, patients were enrolled from 2009 to 2018. Overall consecutive 563 patients (344 ACP vs 219 Amulet) were analyzed and were followed-up for a mean duration of 2.9 ± 1.6 and 1.9 ± 0.9 years. A composite of major peri-procedural complications and major bleedings was assessed as a primary safety endpoint, and stroke, systemic embolism, and cardiovascular/unexplained death were evaluated as the primary efficacy endpoint. To determine the net clinical benefit, they assessed a combination of all the above-mentioned hazards. With regard to efficacy, safety, and net clinical benefit, similar outcomes in the long term were provided by left atrial appendage closure with first and second-generation Amplatzer devices. Not the design modifications of the Amulet but implantation technique and hemodynamics were suggested as factors that may determine clinical results.
