Locking-plate fixation of proximal humerus fractures in patients over 60 continues to be associated with a high complication rate
Journal of Shoulder and Elbow Surgery Jun 27, 2020
Barlow JD, Logli AL, Steinmann SP, et al. - This study was attempted to assess whether and how locking-plate fixation of proximal humerus fractures in patients over 60 continues is correlated with a high complication rate Researchers treated 173 proximal humerus fractures (PHF) in patients > 60 at the institution with open reduction internal fixation (ORIF) applying locked plating between 2005 and 2015. They characterized failure as reoperation or radiographic evidence of failure (average follow-up was 6.1 years). The data indicated that in patients > 60, ORIF of PHFs with locking plates resulted in a 44% complication and 34% failure rate. In older patients and more complex fractures, there was a trend toward higher complication and failure rates. It was demonstrated that refinement in fixation methods and indications are required to optimize the surgical management of PHFs.
