Locking plate fixation of proximal humerus fractures in patients older than 60 years continues to be associated with a high complication rate

Barlow JD, Logli AL, Steinmann SP, et al. - Researchers performed this study to evaluate whether locking plate fixation of proximal humerus fractures in patients older than 60 years continues to be associated with a high complication rate. This study included a sum of 173 proximal humerus fractures (PHFs) in individuals aged > 60 years who were treated at the institution with open reduction and internal fixation (ORIF) applying locking plates. Failure was determined as reoperation or radiographic data of failure. Six-point one year was the average follow-up. In individuals aged > 60 years, ORIF of PHFs with locking plates appeared in a 44% complication and 34% failure rate. In older individuals, there was a trend toward higher complication and failure rates and more complex fractures. The data indicated that refinement in fixation techniques and indications are necessary to optimize the surgical management of PHFs.
