Liver transplantation for acute‐on‐chronic liver failure predicts post‐transplant mortality and impaired long‐term quality of life

Goosmann L, Buchholz A, Bangert K, et al. - Since the role of liver transplantation (LT) in the management of acute‐on‐chronic liver failure (ACLF) is unknown, researchers sought to evaluate the influence of ACLF on post‐LT survival and long‐term graft function, morbidity, and quality of life (QoL). All cirrhosis patients undergoing LT at the institution were analyzed retrospectively between 01/2009–12/2014. Of 250 LT recipients, 98 met the EASL diagnostic ACLF criteria before LT (“ACLF‐LT”). ACLF anticipates impaired post‐LT survival. Although in ACLF and non-ACLF LT survivors, long-term graft function and extrahepatic comorbidities are comparable, the strikingly low QoL in many ACLF-LT recipients warrants attention during follow-up patient care.
