Live births following fertility preservation using in-vitro maturation of ovarian tissue oocytes

Segers I, Bardhi E, Mateizel I, et al. - Researchers investigated the utility of oocytes extracted from excised ovarian tissue and matured in vitro, as adjunct for urgent fertility preservation (FP). Between January 2012 and May 2019, they conducted a retrospective cohort study at a university hospital-affiliated fertility clinic including 77 patients who underwent unilateral oophorectomy for ovarian tissue cryopreservation (OTC). In vitro maturation (IVM) was performed of cumulus-oocyte complexes (COCs) obtained during ovarian tissue processing for 28–42 h. Vitrification or insemination for embryo vitrification was performed of oocytes reaching metaphase II stage. Sixty four patients had vitrification of oocytes (6.7 ± 6.3 oocytes per patient) and 13 patients had ICSI of mature oocytes after IVM, with 2.0 ± 2.0 embryos vitrified per patient. Return of 12 patients to the clinic with a desire for pregnancy was reported. The live birth rate per patient after ovarian tissue oocyte in-vitro maturation (OTO-IVM), ICSI and embryo transfer was 43% in this small series. Findings support OTO-IVM in combination with OTC as a valuable adjunct technique for FP. Further, researchers here described the first live birth after the use of this appended technique combined with oocyte vitrification.
