Late outcomes after endovascular and open repair of large abdominal aortic aneurysms

de Guerre LEVM, Dansey K, Li C, et al. - Researchers conducted a retrospective analysis of prospectively retrieved Vascular Quality Initiative data linked to Medicare claims in order to determine the correlation of large AAA diameter with late outcomes in patients undergoing open and endovascular AAA repair. Between 2003 and 2016, they identified conduction of 21,119 aneurysm repairs. Of these, 15.2% were for large AAAs. Patients undergoing large AAA EVAR had higher 5-year adjusted reintervention, ruptures, mortality, and loss to follow-up rates compared with those having smaller AAA EVAR and large AAA open repair. Hence, they strongly emphasize considering open repair in patients with large AAAs who are medically fit. In addition, these findings emphasize the requirement for rigorous long-term follow-up after EVAR.
