Laparoscopic surgery vs radiofrequency ablation for the treatment of single hepatocellular carcinoma ≤3 cm in the elderly: A propensity score matching analysis

Conticchio M, Delvecchio A, Ratti F, et al. - In this multicentric retrospective analysis of 184 elderly patients (aged >70 years) with single ≤3 cm hepatocellular carcinoma (HCC), researchers compared outcomes between 86 patients who underwent laparoscopic liver resection (LLR) vs 98 who received radiofrequency ablation (RFA). Propensity score matching was applied, after which, 1- and 3-year overall survival rates were estimated to be 96.5 and 87.9% for the LLR group, and 94.6 and 68.1% for the RFA group respectively. For the LLR group, 1- and 3-year disease-free survival rates were estimated to be 92.5 and 67.4%, and these estimates were 68.5 and 36.9% for the RFA group. According to findings, LLR was associated with a longer length of hospital stay and operative time but promised not only a comparable postoperative course but also a better overall and disease-free survival among elderly patients suffering from single HCC (≤3 cm), located in anterolateral segments.
