Labral morphology and number of preoperative dislocations are associated with recurrent instability after arthroscopic bankart repair

Vaswani R, Gasbarro G, Como C, et al. - Researches sought to explain a process to measure capsule and labral volume on preoperative MRI to predict surgical failure after primary Bankart repair. They enrolled a total of 289 individuals who had an arthroscopic Bankart repair between 2006 and 2015. They performed a retrospective case-control study on individuals undergoing primary anterior arthroscopic shoulder stabilization. They characterized surgical failure as a recurrent dislocation event. They examined preoperative magnetic resonance arthrograms by 2 trained reviewers using Vitrea software to measure labral and capsular volume with a 3-dimensional model. This research offers a novel technique of measuring labral and capsule volume with high interrater reliability. There was an association between an increased number of recurrent dislocations prior to primary Bankart repair and increased odds of recurrent instability after surgery. Furthermore, the OR for failure also raised with an increasing number of preoperative dislocations. The found that diffusely small labral morphology was correlated with having a postoperative redislocation.
